I need to submit a Resolution Statement with the AZ Superior Court. It states '...submit a written Resolution Statement as prescribed by Rule 98, Form 4 or 5 as applicable....' I can't find a definition of Rule 98 or what forms are required anywhere. Can anyone point me in the right direction?
